Via Care Health Homes Program

 

Via Care’s Health Homes Program (HHP) serves our chronically ill Medi-Cal members with care coordination, care management, health services, health promotion and family support, and referrals to community and social services.


Services are LGBTQ-friendly and Spanish-speaking. All are welcome.

Eligibility & Services

 

Eligibility

To be eligible for the HHP, you must be a Medi-Cal beneficiary with chronic health challenges such as substance abuse, asthma, diabetes, or heart disease. In addition to comprehensive case management, HHP patients have access to all of Via Care’s on-site services, including primary care medical, dental and behavioral health.

 

Services

Care Management provided by Via Care’s multidisciplinary team includes a comprehensive, individualized care plan for each HHP patient that covers physical, mental, and oral health, substance use disorders, palliative care, trauma-informed care, and social supports.

 

Care Coordination includes working with a Care Manager to address barriers to care, support treatment adherence, reach healthcare goals, and coordinate and integrate services. When necessary, Care Managers accompany HHP members to medical appointments.

 

Health Promotion services include learning healthy lifestyle choices, how to manage chronic disease, and skills development to independently identify and access resources for chronic disease care and prevention.

 

Individual and Family Support Services help HHP patients identify supports, educate family members and care providers about chronic conditions, evaluate needs, and determine how we can work together to continue to improve your health.

 

Community and Social Supports are identified by the Via Care HHP program team to link patients to housing, transition, and tenancy support services for homeless members, as well as to local food and nutrition programs, childcare, and more.

This project is funded in part by L.A. Care Health Plan and will benefit low-income and uninsured residents of Los Angeles County.
