Leaving incarceration is a powerful step toward a new chapter—and you don’t have to take it alone. At Via Care, we understand that reentry comes with real challenges, from accessing healthcare to finding stability and support.
Our Reentry & Justice-Involved Services program is here to help you reconnect to care, rebuild your health, and move forward with confidence. We offer compassionate, judgment-free services—including medical, dental, and behavioral health care—along with support for everyday needs like housing, transportation, and insurance.
Some members of our team have lived this experience themselves, so you’ll find guidance that’s not only professional—but personal.
This program supports individuals who are transitioning back into the community after incarceration.

Access to care is a key part of a successful transition. Our team can help you apply for Medi-Cal, reinstate your coverage, and understand your benefits so you can get care when you need it.
California continues to expand programs that support people returning to the community by improving access to healthcare and care coordination before and after release.
We’re here to make that process easier—so you can focus on your health and your future.
At Via Care, we believe everyone deserves access to quality, compassionate healthcare—no matter their past.
We don’t just provide care—we build relationships and support your long-term success.
