What is Enhanced Case Management (ECM)?

Managing your health can sometimes feel overwhelming—keeping track of appointments, medications, specialists, and even everyday needs like food or housing. That’s where Enhanced Case Management (ECM) comes in. 

ECM is a program designed to help Medi-Cal patients get the support and resources they need to stay healthy. Our care coordinators work closely with you every day to: 

  • Schedule and follow up on medical appointments 
  • Arrange transportation so you can get to care 
  • Connect you with community resources like food, housing, and support services 
  • Help you navigate Medi-Cal benefits so you can access the care you’re entitled to

Who is eligible for ECM? 

  • Must be a Medi-Cal patient 
  • May have multiple chronic health conditions 
  • May have frequent hospital or emergency room visits 
  • May need help accessing community resources, housing, or transportation
