Reentry services for a smooth transition.
Via Care Reentry Program
For justice-involved individuals reentering our community, Via Care offers case management, medical and dental services, behavioral health counseling, hepatitis C screening and treatment, HIV testing and prevention (PrEP and PEP), and linkage to support services.
Services are LGBTQ-friendly and Spanish-speaking. All are welcome.

Medi-Cal, My Health L.A. and all health insurance accepted. Sliding fee available for uninsured and cash patients. Via Care accepts all patients regardless of insurance or financial status.
Via Care Reentry Services
Case Management and Social Services
Social services navigation is provided to clients by trained Community Health Workers (CHWs), who themselves have prior histories with incarceration. Clients are linked to transportation, housing and employment, needle exchange, food, child care, education and more.
Medical and Dental Services
Medical and dental services are available to all reentry clients including; physicals, acute care, chronic disease screenings and care, family planning, immunizations, nutrition, prescriptions, dental examinations and x-rays, cleanings and deep cleaning, crowns, bridges, dentures, fillings, extractions and root canals.
Behavioral Health Counseling
Behavioral health services include counseling for anxiety, depression, bereavement, PTSD, parenting, sexual abuse, neglect and abuse, and substance use disorders. Via Care offers both one-on-one and group counseling.
HIV and STI Testing and Prevention
HIV and STI services include free rapid HIV testing, case management, and specialty medications to protect clients before HIV exposure (PrEP), and if exposed, emergency HIV prevention treatment (PEP). STI testing and treatment is available to all clients on-site at Via Care.
Hepatitis C Testing & Treatment
Hepatitis C screening (HCV) is available with a simple blood test. For HCV-positive patients, Via Care offers a 6-month treatment regimen under the care of a medical provider. Patients may experience positive recovery results, and many patients are cured completely.
